If You Are Late For Your Appointment
We will try to work you back into the schedule, or perhaps offer you an appointment with another provider.
Out of courtesy to our other patients, we may be forced to ask you to reschedule your appointment.
ADHD Consultation or Follow-Up
ADHD Initial Consultation
If you or your child’s teacher are concerned about your child’s inattentive, hyperactive, or impulsive behaviors, please call our office to schedule an ADHD evaluation.
Please complete the ADHD Initial Evaluation Packet prior to your appointment. For children in 6th grade and beyond, please have 2 teachers ( Science or Math and ELA or Social Studies) complete the Vanderbilt Teacher Initial Form. Please do not bring any sibling to the visit.
Important! We request assessment forms be available to the provider for review 5 days prior to your child's appointment.
ADHD Follow-Up
After your child's initial evaluation, if medication is started, your child must be seen within 21-28 days in order to continue the medication.
If your child is doing well with his/her medication, subsequent appointments will be scheduled every 3 months.
If a medication or dosage change is needed, your child must be seen within 1 month to assess your child's progress and to determine if the medication can be refilled.
If medication was stopped for more than 4 months and restarted, your child must be seen within 1 month.
Anxiety/Depression Consultation
In cooperation with community based mental health providers, we evaluate and treat children, teen and young adult with anxiety and depression concerns.
Please complete the age-appropriate evaluation forms.
Anxiety
Assessment tools for anxiety for 8-12 years old:
- Screen for Child Anxiety Related Disorders (SCARED) - Parent
- Screen for Child Anxiety Related Disorders (SCARED) - Child
- Both you and your child will need to complete the appropriate SCARED assessment.
Assessment tools for anxiety for 13 years old and older:
Depression
Assessment tools for depression for age 6-12 years old:
- Center for Epidemiological Studies Depression Scale for Children (CES-DC)
Assessment tools for depression for age 12-18 years old:

Anxiety/Depression Follow-Up
If the provider prescribes medication, a 7-day telephone or MyChart follow-up is expected to assess for any side effects and to answer any concerns/questions you may have.
An in-person follow up within 21-28 days will be required to assess your child's progress and to determine if the medication can be refilled.

Asthma
Children with asthma should be seen in our office every 3-12 months depending on their severity of illness and their asthma control. Regular visits are required in order for us to evaluate your child prior to medication refills.
Please remember to bring all the asthma medications to the visit and complete the age appropriate Asthma Control Test (ACT) based on your child's age.
Behavioral/Developmental Consultation
f you have concern about your child's behavior or is wondering if your child is achieving the age appropriate developmental milestones, you can schedule a visit with one of our providers.
Some of the behavioral concerns are aggression, dysregulated moods, separation anxiety, impulsivity, tantrums.
Sick Child Visits
MORE ON SICK CHILD VISITSWe make every effort to schedule appointments for ill children on the day that you call us. Please call our office ahead of time so that we can minimize the time that you will spend waiting.
If you have a medical emergency during office hours, we will do our best to see your child immediately or refer you to the appropriate facility.
Telehealth
MORE ON TELEHEALTHTelehealth appointments are typically scheduled at the end of the morning or the afternoon shift.
PLEASE NOTE: An active MYCHART Account is needed for ALL Virtual Appointments. Patients age 13 and older MUST have their own MYCHART Account for our front desk to schedule a Virtual Appointment.
Well Child Visits
WELL CHILD VISITS SCHEDULEDid you know regular well child visits are one of the most important things you can do to keep your child healthy? Well child visits also provide you with the opportunity to discuss any questions or concerns you may have about your child’s health.
Your child should have a well child visit at the intervals listed below. We recommend that you schedule your next appointment at the time of check in. Please call our office 3 months in advance to schedule an appointment with your child's primary care provider on the date that best fits your needs.
